INSURANCE
I do not participate with any insurance. I will provide a statement for fees at each visit which you can submit to your insurer for reimbursement
FEES
First appointment: $350 for 2 hour evaluation
Medication management 30 minute appointment: $150
Individual therapy 50 minute appointment: $250
Prescriptions refills between sessions: $25
PAYMENT METHODS
I do accept checks, cash and credit cards. With credit cards there is an approximately 3% processing fee added to the bill
ADMINISTRATIVE FEES
Missed appointments: I do charge for the full session unless there is a cancellation 2 business days before the appointment
Returned checks: $85 plus the fee for the visit
